A survey on in-patients' demands of continuing care after discharged

Objective To investigate in-patients' demands of continuing care after discharged.Methods 216 in-patients from different clinical wards were surveyed by questionnaire. The data were analyzed by description and correlation analysis methods. Results 81.94% in-patients expressed that they still need continuing care after discharged. The top of six needs were health information, psychological counseling, basic nursing, specific nursing, consulting hours and basic disease related knowledge. Popular methods of care services ranked from highest to lowest were provision of literal material, individual guidance, lecture,communication between fellow patients and providing video and recorded data. Conclusions Discharged patients still need well targeted, personalized and continuing care services. Key words: Care needs;  Discharged;  Survey
