Eficienţă şi eleganţă versus complicaţii - „banda în H” în tratamentul prolapsului de perete anterior vaginal şi al incontinenţei urinare de efort

Our goal is to report the preliminary results of trans­­ob­tu­rator four-arm polypropylene mesh in the treatment of high-stage anterior vaginal wall pro­lapse and con­co­mitant stress urinary incontinence (SUI). The study included 67 patients who underwent anterior vaginal wall repair with polypropylene mesh in Obstetrics and Gynecology Clinic, Elias Universitary Hospital Bucharest, between June 2012 and July 2015. Failure rate was: 1 case (1.49%) of cystocele recurrence diagnosed at one month, subjective recurrence in 4 patients (5.97%). Regarding SIU: 65/67 patients reported significant improvement; 2 patients - slight improvement or none. Immediate postoperative complications were minor - ecchymosis of cuttaneous incisions 18/67; dysuria 11/67. Late complications were inguinal and vaginal pain (13/67 - 19.4%).  Once the technique is well established, we consider that the four-arm polypropilene mesh is an elegant method to treat anterior vaginal wall prolapse and SUI, providing good results on short and medium term.
