The increasing frequency of video-assisted thoracoscopic interventions, as well as open thoracic surgical procedures in children, demands appropriate anesthetic techniques to provide single-lung ventilation. Techniques for single lung ventilation in children have included several devices because they all have their individual limitations and one must adapt to the possibilities routinely available in the institution. Successful use of Fogarty occlusion embolectomy catheters as bronchial blockers for lung isolation and single-lung ventilation has been documented using different techniques. Placement is either performed blindly or guided with visual confirmation using the bronchoscope or fluoroscopy. We describe the successful placement of a Fogarty catheter for single-lung ventilation through a single lumen tube in a 2 year old boy proposed for a lower right pulmonary lobe resection that allowed us an uneventful ventilation throughout the surgical procedure.
