MRCP and ERCP: a comparison in 45 patients

Background MRCP is a recent non-invasive method that provides high-resolution images of the biliary and pancreatic ducts and will probably replace diagnostic ERCP. This paper aims to evaluate MRCP diagnostic accuracy compared with ERCP. Methods A prospective study was undertaken of 45 patients in the age range 32–75 years. MRCP preceded ERCP by an average of 3 days. Results Patients were divided into four groups: normal, common bile duct stones, biliary stricture and chronic pancreatitis. MRCP was successful in 44/45 patients (98%), while ERCP failed in three patients (93% success rate). The MRCP diagnosis was correct in all but one patient (98%). Overall diagnostic accuracy was 92%. 13/41 (32%) patients had a normal ERCP. If MRCP were the first choice for evaluating such patients it would be possible to save time and financial resources, and to avoid complications. Discussion Patients with suspected biliary or pancreatic disease, but without clear signs of stricture on abdominal US scan, should be referred for MRCP.
