Urinary tract infection (UTI) is an infection with the most common bacterial involvement in the community and nearly 10% of people have been exposed to UTI during their lifetime. Approximately 150 million people worldwide are diagnosed each year with urinary tract infections. The purpose of this study was to determine the appropriateness of antibiotic use with hospital formulary in patients of Asy-Syifa Hospital Boyolali in 2016. This research is a descriptive study with retrospective data collection. Data taken from Medical Record at Asy-Syifa Hospital Sambi Boyolali in 2016. The results showed that the type of antibiotic used at the Asylum Syifa Hospital Boyolali in 2016 was the use of a single antibiotic Ceftriaxon (10.14%), Cefoperazon (1.44), Cefarox (5.79%), Cefixime (2.89%), Levofloxacin (1.44%), Ciprofloxacin (2.89), Use of antibiotics 2 combinations Ceftriaxon & Cefixime (49.27%), Ceftriaxon & Ciprofloxacin (7.24%), Cefriaxon & Levoflocaxin (1.44%), Cefixime & Cefoperazon (10.14%), Cefixime & Cefotaxim (1, 44%), Cefixime & Ciprofloxacin (1.44%), Cefarox & Ceftriaxon (1.44%), Ceftriaxon & Cefoperazon (1.44%), Cefarin & Cefarox (1.44%).
