Pulmonary Hypertension in Sarcoidosis

In a patient with a 25-year history of uncomplicated sarcoidosis, progressive right ventricular failure developed in the absence of clinical evidence of end stage, lung disease. At autopsy, a predominantly perivascular granulomatous process with minimal alveolar involvement was found. This case demonstrates that sarcoidosis can be associated with severe pulmonary vasculitis in the absence of extensive parenchymal involvement. Cor pulmonale and death can result from this complication of sarcoidosis.
