The results of 45 radical cystectomies with urine derivation to sigmoidorectal reservoir by Mainz-Pouch II have been analyzed. The majority of the patients were elderly and old people with locally advanced and disseminated forms of urinary bladder cancer. Postoperative complications developed in 15,5% cases, for 4,4% of the patients it was the reason of lethal outcome. Having analyzed the character of complications it was detected that generally they were connected with concomitant diseases and prevalence of cancer process, but not with the method of urine derivation. 3-year long probability of survival of patients has been 63%. The quality of patients' life has proved to be comparable with the same data taken after urine derivation to orthotopic urinary bladder. Radical cystectomy with urine derivation to sigmoidorectal reservoir is operation of choice for the patients, for whom it is impossible to form orthotopic urinary bladder cause of some reasons.
