The total in blood of bilirubin in newborns of 35 weeks gestation o more is in greater than 25 mg/dL, it is considered a medical emergency and the exchange transfusion should be performed immediately. This should only be performed by trained personnel in the intensive care unit, always weighing the benefits against the risks. The case of a new born of 40.4 weeks of age gestation is informed with ABO hemolytic disease treatment with exchange transfusion with used old reconstituted blood of 30 days. The exchange transfusion is a procedure with high morbidity and mortality in the management of the hemolytic disease in the new born. To minimize the complications mainly of metabolic type it is recommended when used fresh (< 7 days old) reconstituted blood. The case of a new born of 40.1 weeks of age gestation is informed with Rh hemolytic disease treatment with exchange transfusion with used old reconstituted blood of 30 days.
