Traumatic Aneurysm of the Proximal Anterior Cerebral Artery

Traumatic cerebral aneurysms are rare in severely head-injured patients. We present the first case of a traumatic aneurysm of the A1/A2 junction. A 16-year-old young man was injured in a motorcycle accident. He was operated on for an epidural hematoma; additionally the cranial CT-scan revealed fractures of the fronto- basis. On the 11th day he suffered from a massive subarachnoid hemorrhage. Cerebral angiography was performed, revealing a traumatic aneurysm of the right A1/A2 junc- tion. Clipping was not possible because of the severe brain edema and the patient's poor status. The patient died 36 hours after the diagnosis. In case of delayed neurological deterioration in brain-injured patients a traumatic cere- bral aneurysm should be ruled out.
