Association of Motor and Non Motor Symptoms Severity with Motor Laterality Asymmetry in Parkinson’s Disease (P3.8-044)

Objective: The aim of this study was to evaluate the predominant laterality of motor symptoms and its relationship with cognition and other Non Motor symptoms (NMS) in Parkinson’s disease (PD). Background: In PD asymmetric motor signs is intriguing and provide a model to evaluate whether asymmetric nigrostriatal degeneration affects the severity of motor symptoms, and NMS. Design/Methods: Nationwide, cross-sectional, multicenter (Coppadis Registry) outpatient PD study. Left PD (LPD) and right PD (RPD) was defined based on the motor UPDRS scale. To include the clinical spectrum of asymmetric PD patients, we considered two groups of patients with mild-moderate and extreme asymmetry. Predominant LPD or RPD with mild-moderate versus extreme asymmetry were compared in terms of dopaminergic drug doses, motor severity (motor UPDRS in the On, Off states), cognition [Parkinson’s Disease Cognitive Rating Scale [PD-CRS], NMS [Non-Motor Symptoms Scale (NMSS)], depression-anxiety [(Beck Depression Inventory-II (BDI-II)], Impulse control disorders (QUIP-RS), and quality of life (PDQ-39) scores. Statistical analysis included non parametric comparisons and regression analysis. Results: 694 PD patients (62.2 ± 8.8 years 57.8% males), 307 (44.2%) RPD, 345 (49.7%) LPD, 42 (6.1%) symmetric PD were included. Compared to RPD, LPD had similar age, disease duration, PD-CRS, BDI-II, QUIP-RS scores and levodopa equivalent dose, but higher scores in the UPDRS motor in the Off (p Conclusions: These findings suggest that damage to right-hemisphere plays a disproportionately greater role in PD-related motor and overall NMS severity. Better understanding of the mechanisms contributing to lateralization of neurodegeneration will help to develop more targeted therapeutic approaches, taking into account subtypes of PD as a heterogeneous disorder.
