Clinical application of laparoscope-assisted left hemicolectomy for colon carcinoma

2014
Objective To compare the radical effect, the incidence rates of postoperative complications and outcomes between laparoscope-assisted left hemicolectomy and conventional laparotomy for colon carcinoma. Methods The data of 45 patients who underwent left hemicolectomy in Henan Tumor Hospotal from October 2006 to May 2010 were retrospectively analyzed. The patients were randomly divided into two groups.Laparoscope-assisted left hemicolectomy group consisted of 23 patients, conventional laparotomy group consisted of 22 patients. The radical effect, the incidence rates of postoperative complicatio-ns and outcomes in these two groups was compared. Results Laparoscope-assisted left hemicolectomy group were similar to conventional laparotomy group in lymph node harvest, the length of specimen removed, the incidence of postoperaive complications, the 3 years survival rate, the 3 year disease-free survival rate. The mean operation time of laparoscope-ass-isted left hemicolectomy group [(176 ± 24) min] was more than that of conventional laparotomy group [] (148 ± 15) min [,the difference was statistically significant (P <0.05).The blood loss [(63 ± 22) ml],the mean time for passage of flatus [(3.3 ± 1.0) d],the average length of stay [(10.8 ± 1.8) d],the postoperative catheterization time [(2.1 ± 0.7) d],the time to diet [(5.0 ± 1.5) d],the length of abdominal incision [(6.2 0.8) cm] and the time of analgesia (1.9 ±0.7)d of laparoscope-assisted left hemicolectomy group much better than those of conventional laparotomy group(P <0.05).Conclusions Laparoscope-assisted left hemicolectomy for colon carcinoma conforms to the principle of radical cure of carrinama and is a promising option.
