Atividades farmacêuticas em um projeto social aplicadas em unidades de saúde básica: um relato de experiência

Self-medication in conjunction with the irrational use of medicines has become a worldwide problem. Using drugs in correctly can mask symptoms and accelerate the worsening of the disease. Therefore, several health professionals assume an important role in guiding the population, where the pharmacist assumes a fundamental role, helps in the promotion and guidance on the adequate an drational use of medicines, safety, quality and treatment in the treatment of the population. This project presents the creation and application of a pharmaceutical care form that will investigate how pharmaco therapies and the use of medicines by patients present in the project. This work focuses on reporting an academic pharmacy experience in a social project that took place in the interior of Piaui.
