Отдаленные результаты консервативного лечения стрессовых переломов костей нижних конечностей

2019
Objectives : To evaluate long term results of lower limbs stress fractures conservative management. Material and methods : 55 patients were included after stress fracture conservative management in European Clinic of Sports Traumatology and Orthopaedics (ECSTO) from 2010 to 2016. The average age accounted 37 years (range 15 to 65). 65% (36) were female and 35% (19) – male. Patients were assessed in long term with subjective forms FAAM (ADL + sport) and LEFS. Results : The median time from trauma to follow up subjective assessment accounted 3 years (range 1 to 7 years). Stress fracture 2 and 3 metatarsal bone was registered most often (25, 47%) The most common reason for stress fracture was jogging (38%), second most common reason - walking (29%), while game sports and fitness acounted18% and 9% respectively. The median FAAM ADL score accounted 100% (IQR from 96 to 100), the median FAAM sport score - 100% (IQR from 91 to 100). As assessed by LEFS scale excellent results were obtained in 87% of cases (48), good – in 11% of cases (6), fair – in 2% (1). Follow up MRI evaluation according to E.A. Arendt et al. classification revealed grade 2 changes in 18% of cases (10), grade 3 in 26%(14), grade 4 in 29%(16). Conclusion : Jogging appeared to be the most common reason for lower limb stress fractures. This type of bone reaction can be managed conservatively by restriction of weightbearing, physio, NSAIDs and individual insoles.
