Скрининг лиц 60 лет и старше в Семейском регионе на наличие инфраренальной аневризмы брюшной аорты, определение тактики их лечения

2011
The aim of the work: to develop a strategy for early detection of patients with abdominal aortic aneurysm (AAA) and the tactics of surgical treatment of concomitant coronary artery lesions, brachycephalic arteries and lower limb arteries. 1011 people aged 68.4?1.2 years were surveyed. 363 (35.9%) men and 648 (64.1%) women. Along with clinical examination the ultrasound dopplerography (USDG) of abdominal aorta with subsequent angiography of the problem vascular systems were performed. Some patients underwent surgical correction of the AAA and revascularization of arteries of lower extremities. The frequency of AAA 52 (5.1%) cases and 98.1% of identified aneurysms were smaller size (3-5 cm in diameter). Concomitant affection of coronary arteries and AAA was observed in 24 (57.1%) cases, combination of AAA and brachiocephalic artery stenosis was diagnosed in 19 (45.2%) patients, affection of lower limb arteries in 25 (48.1%) cases. Conclusion. For people older than 60 years it is advisable to make USDG of abdominal aorta in addition to ultrasound of peripheral arteries. Early identification of patients with AAA creates favorable conditions for the timely correction of associated lesions of the coronary and brachiocephalic arteries and significantly reduce the frequency and severity of possible complications, and finally improve the results of surgical treatment.
