Case study: lessons learned from the failure of a new 230-kV transformer-cable termination

Investigation of the failure of a new 230 kV transformer-cable termination showed that a micro-crack had developed in the bushing during transportation/handling. It is suggested that commissioning tests should include partial discharge measurements.
