Histerectomías laparoscópicas en el hospital Dr. Luis Razetti de Barinas

Mostrar la experiencia en histerectomias laparoscopicas en el Servicio de Cirugia General del Hospital “Dr.Luis Razetti” de Barinas. Periodo agosto 2013- septiembre 2015. Estudio observacional descriptivo, de corte transversal. Se incluyeron pacientes que aceptaron elprocedimiento por via laparoscopica, con patologias benignas o consideradas pre malignas que justificaran la cirugia.Las variables pertinentes fueron obtenidas de la revision detallada de las historias clinicas de las pacientes. Se intervinieron 20 pacientes, edad promedio 43 anos, 60% con antecedentes de cirugias pelvicas, 100%con diagnostico de miomatosis uterina, 7 (35%) con diagnosticos asociados y 4 (20%) ameritaron otros procedimientosquirurgicos. Se realizaron 13 (65%) histerectomias laparoscopicas tipo IV y 7 (35%) tipo I.La media de sangradotransoperatorio fue 137,50 cc, el tiempo quirurgico promedio fue 139 minutos con reduccion de 35% con el avance dela curva de aprendizaje; el tiempo promedio de hospitalizacion fue 22 horas. Hubo complicaciones en 3 casos, todaspostoperatorias y sin necesidad de reintervencion o nueva hospitalizacion. Las tecnicas de histerectomias laparoscopicas son seguras, reproducibles y ofrecen ventajas enterminos de recuperacion, dolor y complicaciones, suponen una disminucion de costos relacionada con la menorestadia hospitalaria. Sin embargo, la necesidad de equipos especializados es una limitante que puede ser dificil desortear en el medio publico donde se realizo el estudio. To show the experience in laparoscopic hysterectomies at the General Surgery Service of the “Dr. Luis Razetti“Hospital of Barinas. Period August 2013 to September 2015. Observational, descriptive, cross-sectional study. Patients who accepted the procedure by laparoscopy wereincluded, with benign pathologies or considered pre malignant to justify the surgery. The relevant variables were obtainedfrom the detailed review of the patient’s medical records. Twenty patients were intervened, average age 43 years, 60% with pervious pelvic surgeries, 100% with diagnosisof uterine myomatosis, 7 (35%) with associated diagnoses and 4 (20%) merited other surgical procedures. There were 13(65%) laparoscopic hysterectomies type IV and 7 (35%) type I. The average of transoperative bleeding was 137.50 cc;the average surgical time was 139 minutes with a reduction of 35% with the advance of the learning curve; the averagehospitalization time was 22 hours. There were complications in 3 cases, all postoperatively and without the need forreoperation or new hospitalization. Laparoscopic hysterectomy techniques are safe, reproducible and offer advantages in terms of recovery, painand complications; they represent a reduction in costs related to shorter hospital stay. However, the need for specializedequipment is a limitation that may be difficult to overcome in the public environment where the study was conducted.
