This chapter summarizes eye movement disorders characterized by intact alignment, in which the eyes have restricted motility, move too slowly, or are misdirected. This condition tends to result from impaired supranuclear input upon the ocular motor nuclei. The relevant anatomy of the ocular motor system is discussed first, followed by signs and symptoms in patients with this type of diplopia. An extensive differential diagnosis of the diseases that can cause this syndrome is made.
