Repair of segmental bone defects with OSTEOSET DBM for osteofibrous dysplasia

2009
Objective To investigate the therapeutic effects of postoperative clinical manifestations and roentgenographic findings in patients with osteofibrous dysplasia treated with OSTEOSET DBM (Demineralised Bone Matrix) in repairing segmental bone defects. Methods From Jan 2004 to Jun 2006, 13 patients with osteofibrous dysplasia were enrolled in this study, including 8 boys and 5 girls, with the mean age of 10. 5 years ranging from 5 to 16 years. Six patients were diagnosed with a lesion in a single bone (femur or tibia) and 7 with multi-lesions. The patients underwent long segmental resection and their affected bones were packed with OSTEOSET DBM (Wright Medical Group, Inc. ) to fill segmental defects, followed by the fixation with plates and screws. Results All patients were followed up for 12 to 42 months with the mean period of 28 months. Clinical manifestations and roentgenographic findings were evaluated. The internal fixators were removed in 24. 2 months, ranging from 18 to 36 months. The bone materials (OSTEOSET DBM) were absorbed and substituted by bone tissues gradually, achieving clinical healing at 12 to 18 months postoperatively. Three years later, the neogenetic bones integrated mutually with mature lamellar bone and bone trabecula. Mean- while, the medullary canal reunioned, and a new canal as a whole was formed. No tumor recurrence or disunion was observed. Conclusions Repair of segmental bone defects with OSTEOSET DBM for osteofibrous dysplasia can achieve good therapeutic effect. As a satisfactory implantation substitute, OSTEOSET DBM can accelerate bone reparation, which is beneficial to vascular ingrowth and can promote bone reunion. OSTEOSET DBM may become a satisfactory therapeutic alternative for patients with osteofibrous dysplasia after resection long segmental. Key words: Osteofibrous dysplasia;  Excision;  Bone defect
