Thyroid Pyramidal Lobe Mimicking Parathyroid Adenoma in (superscript 99m)Tc-MIBI Parathyroid Scintigraphy

The (superscript 99m)Tc-MIBI parathyroid imaging is useful for evaluation of hyperparathyroidism. False-positive findings have been reported in cases with thyroid nodular goiter, adenoma, and carcinoma. However, functioning thyroid remnant, especially stimulated by high TSH level, has not been mentioned. The uptake in hyperplastic pyramidal lobe deserves special attention in patients after thyroid surgery.
