The diagnostic value of urinary trypsinogen-2 in screening acute pancreatitis

2005
Objective To explore the diagnostic value of urinary trypsinogen-2 in screening acute pancreatitis. Methods In 280 acute abdominal pain patients, urinary trypsinogen-2 was tested by immunochromatography, serum and urinary amylase was detected by enzymology method.Result Urinary trypsinogen-2 was positive in 53/55 with acute pancreatitis, the sensitivity was 96.4%;12 cases with false positive were found in 225 cases without acute pancreatitis, the specificity was 96.3%.The sensitivity and specificity of serum amylase were 81.8%(45/55) and 88.9%(25/225),respectively.The sensitivity and specificity of urinary amylase were 80.0%(44/55) and 88.4%(26/225),respectively.There were statistical significances in differences of the sensitivity and specificity in above three parameters(P0.05).Urinary trypsinogen-2 was positive in all of 7 patients with severe acute pancreatitis. Conclusion Detection of urinary trypsinogen-2 is a convenient and rapid diagnostic method with high sensitivity and specificity for screening acute pancreatitis in patients with acute abdomen.
