The patellofemoral instability is pathology of the adolescent and young adult. An important contributory factor is trochlear dysplasia, which may cause patellar recurrent dislocation. Trochleoplasty pretends to recreate the patellofemoral biomechanical stability. The authors present a case report of a young woman, combat sports practitioner, with trochlear dysplasia and patellar recurrent dislocation. The patient did a trochleoplasty and was submitted to a rehabilitation programme, with good outcome. In this case report diagnosis, treatment and follow-up results are discussed.
