In contrast to elective surgery for infrarenal abdominal aneurysms the mortality rate of ruptured aneurysms has not improved over the years. Own experience and a review of the literature suggest three basic goals for achieving lower mortality: 1. Reducing the time interval between rupture and the surgical control of the bleeding, 2. refraining from reestablishing normal blood pressure by hemodilution, 3. admitting the patient to a competent surgical unit with experience in aortic surgery.
