Establishing a System for Ensuring Transfer Completion

Preparing a young adult for the eventual transfer from pediatric to adult-centered medical care is a complex process that can be successfully accomplished with appropriate planning. The process can be aided with the development of a transition care consult service. However, the successful transfer of a patient is most often achieved by developing a relationship between pediatric and adult providers while improving communication between these providers during this process. It is also incumbent on providers to prepare patients for the transition process and assist in connecting them to adult caregivers. Ultimately, feedback should be obtained from the patient and providers to improve the experience for future individuals undergoing transition and transfer.
