Implementation of a Personal Health Library (PHL) to Support Chronic Disease Self-Management

This paper shows a work-in-progress on the implementation of an integrated Personal Health Library (PHL) for Chronic disease self-management (CDSM) using the Social Linked Data framework. The proposed method is fully decentralized and follows the linked open data platform specifications and grants patients true ownership over their data while giving them fine-grained access control mechanisms to exchange and share their data. It also supports interoperability and portability by following standard protocol, format, and vocabulary provided by ontologies.
