Our philosophy in the treatment of esophageal carcinomas is based on the fact that no other therapeutic method is able to treat dysphagia as well and is as long lasting as surgery. Surgery is the best way to give the patient a better quality of life for the rest of his life. Therefore we are also convinced that performing palliative resections of advanced tumors, is the right treatment. This is also our main argument for performing a transthoracic en bloc resection because with this technique more tumors are resectable [1–4, 8].
