Innovation of Care Service System for Patients Risk with Chronic Kidney Disease: A Case Study of the Community Hospital, Nakhon Pathom Province

Purposes: To study the elements, characteristics, development and management process, outcomes and impacts of nursing innovation on increasing nursing services, reducing health risks, including conditioning factors of the nursing innovation for patients with risk of chronic kidney disease. Design: Qualitative research design: a case study. Methods: Research participants were four key informants, an innovative developer, health care providers and the officer. They worked and participated in the chronic disease clinic at the Community Hospital, Nakhon Pathom Province, where the nursing innovation was developed. In-depth interview, focus group discussion and documentary research were used for data collection. Qualitative data were analyzed by using the Typological analysis method. Quantitative data were presented with percentage. Main findings: Characteristic of this innovation is a process innovation. Key element is a nurse innovative developer who developed and managed this innovation by applying roles of Advanced Practice Nurse and nurse case-manager. The development, management process and the conditioning factors were the network and collaboration from all of the department and the community. The significant outcomes of clients who participated in this innovation were increasing their basic self-care skills, reducing HbA1C level, good controlling blood pressure, reducing the severity of chronic kidney disease, and decreasing the clients’ expense. Conclusion and recommendation: This nursing innovation is a good practice to promoting health behavior of clients in a one district, Nakhon Pathom province. Therefore, the prospective study is recommended to examine the long-term effect of this management which may be develop or apply to another community. นวตกรรมการจดระบบการดแลสำหรบผปวยกลมเสยงโรคไตวายเรอรง: กรณศกษา โรงพยาบาลชมชนแหงหนงจงหวดนครปฐม บทคดยอ วตถประสงค:  เพอศกษาองคประกอบ คณลกษณะ กระบวนการพฒนา และการจดการนวตกรรม ผลลพธและผลกระทบของนวตกรรมกบการเพมคณภาพบรการพยาบาลและการลดปจจยเสยงดานสขภาพของผรบบรการและปจจยเงอนไขของการพฒนานวตกรรมการจดระบบการดแลสำหรบผปวยกลมเสยงโรคไตวายเรอรง รปแบบการวจย: เปนการวจยเชงคณภาพ: กรณศกษาเฉพาะในหนวยงานแหงหนง วธดำเนนการวจย: ผใหขอมลมจำนวน 4 คน ประกอบดวยผพฒนานวตกรรม บคลากรทางสขภาพ และเจาหนาททปฏบตงานในคลนกโรคเรอรงของโรงพยาบาลชมชน จงหวดนครปฐม เกบขอมลดวยการสมภาษณเชงลก การสนทนากลม และขอมลจากเวชระเบยน วเคราะหขอมลเชงคณภาพแบบจำแนกชนดของขอมลระดบมหภาค และวเคราะหขอมลเชงปรมาณดวยสถตรอยละ  ผลการวจย: นวตกรรมนมคณลกษณะเปนนวตกรรมกระบวนการ ซงมองคประกอบทสำคญ คอ พยาบาลผพฒนานวตกรรมทปฏบตบทบาทของการเปนพยาบาลผปฏบตการพยาบาลขนสง และพยาบาลผจดการรายกรณไดอยางดยง โดยมกระบวนการพฒนา การจดการนวตกรรม และปจจยเงอนไข คอ การสรางเครอขายในการสรางเสรมสขภาพจากการไดรบความรวมมอจากหนวยงานและทกภาคสวนในชมชน สงผลใหผรบบรการในพนทมทกษะพนฐานในการดแลตนเอง สามารถลดระดบคานาตาลเฉลยสะสมในเลอด ควบคมระดบความดนโลหต ชะลอความรนแรงและชะลอการเกดโรคไตวายเรอรง รวมทงสามารถลดคาใชจายของผรบบรการไดอกดวย สรปและขอเสนอแนะ: นวตกรรมทางการพยาบาลนเปนนวตกรรมทมการปฏบตทเปนตวอยางทด (good practice) ในการสรางเสรมสขภาพของผรบบรการกลมเสยงโรคไตวายเรอรงในเขตอำเภอหนง ในจงหวดนครปฐม ดงนน จงควรมการศกษาตดตามผลการพฒนาในระยะยาวของวธการจดการนซงอาจนำไปสการพฒนาระบบการจดการโรคเรอรงอนๆ หรอนำไปประยกตใชในชมชนอนๆ ได คำสำคญ: นวตกรรมทางการพยาบาล โรคไตวายเรอรง โรงพยาบาลชมชน
