A CASE OF LOCALIZED MALIGNANT PERITONEAL MESOTHELIOMA

The localized malignant peritoneal mesothelioma is rare. We successfully resected such a tumor formed in the right lower abdomen. A 55-year-old female was seen at the department of gynecology in this hospital because of a right lower abdominal pain and sensation of abdominal fullness. There was no abnormal findings in the uterus and adnexa. But MRI showed a tumor of the cecum or appendix, and she was referred to our department. Her abdomen was generally flat and soft, but an unmovable hard tumor of small fist size exsisted in the right lower quadrant. Digital examination could not detect any tumor. Various imaging diagnoses revealed the tumor in the right lower abdominal wall, and then extirpation of the tumor was performed under general anesthesia. The tumor mainly existed in the preperitoneal fat space and invaded the transversus abdominis muscle. The tumor was resected with the part of transversus abdominis muscle and peritoneum. It was 5.5×5×4cm in size and a solid tumor with central necrosis, and histological diagnosis was the localized epitherial malignant peritoneal mesothelioma. Generally this disease has a poor prognosis. CA125 level that elevated preoperatively, returned to normal after the operation, and there has been no recurrence up to now.
