Kifejlesztesre kerult egy uj meresi modszer, mely alkalmas az NT-proANP es az NT-proBNP egyuttes meresere (NT-proXNP). Cardiopulmonaris bypasson (CPB) atesett gyerekeken vegzett vizsgalatainkban az NT-proXNP szignifikans osszefuggest mutatott az NT-proANP szinttel, de meg szorosabbat az NT-proBNP-vel, illetve a ket NP osszegevel. Az NT-proXNP szint szoros korrelaciot mutatott a cardiac indexszel, a stroke volume indexszel es a globalis EF-fel a postop. periodusban. Az NT-proXNP tehat jo marker a cardiac output utankovetesere gyermekkori szivsebeszetben. Preklinikai vizsgalatunkban az inozinnak a szivre kifejtett hatasait vizsgalatuk reperfuzio soran cardioplegias szivmegallasnal extracorporalis keringesű kutyamodelleknel. Eredmenyeink szerint az inozin alkalmazasa javitja a cardialis es endothelialis funkciot hypothermias szivmegallasban vegzett CPB műtet utan. Tovabbi experimentalis vizsgalatainkat izolalt perfuzios patkanysziven vegeztuk. A myocardiumban korabban kimutatott magas szamu prolactin releasing peptide (PrRP) kotőhelyek kardialis szabalyozasban jatszott szerepet vizsgaltuk. Langendorff szerint izolalt perfuzios patkanysziven (n=99) a PrRP adasa (1-100nM) dozis-dependens pozitiv inotrop hatast fejt ki. Eredmenyeink egy ez idaig ismeretlen szerepet tartak fel a PrRP-nek, miszerint direkt pozitiv inotrop hatassal van a szivizomra. | A novel diagnostic method was developped which incorporates information about the concentrations of both NT-proANP, NT-proBNP. In our children patients undergoing cardiac operation with cardiopulmonary bypass (CPB) the NT-proXNP level correlated significantly with the NT-proANP level, but more strongly with the NT-proBNP and the arithmetic sum of both. NT-proXNP had a strong correlation with cardiac index, the stroke volume index and the global EF throughout the post-operative period. NT-proXNP is a good marker of cardiac output following pediatric cardiac surgery In a pre-clinical study, we investigated the effects of inosine on cardiac function during reperfusion in experimental dogs model of cardioplegic arrest and extracorporal circulation. In conclusions the application of inosine improves myocardial and endothelial function after cardiopulmonary bypass with hypothermic cardiac arrest. In our additional experimental study isolated perfused rat hearts were made. High levels of specific prolactin-releasing peptide (PrRP) binding sites have been found in the myocardium; however, the functional importance of PrRP in the regulation of cardiac function is unknown. In isolated perfused rat hearts, infusion of PrRP (1–100 nM) induced a dose-dependent positive inotropic effect. In conclusion, the present data define a hitherto unrecognized role for PrRP in the regulation of cardiovascular system by showing that PrRP exerts a direct positive inotropic effect.
