Risk factors and prognosis analysis of patients with hepatic metastases from rectal cacer

2010
Objective:To investigate the clinicopathological factors of rectal cancer associated with hepatic metastases. Methods: A retrospective analysis of 494 cases with rectal cancer was designed. All patiens were divided into two groups according to hepatic metastases or not by preoperative or intraoperative detection of liver, and by the way of univariate and multivariate analysis.Clinicopathological factors were analyzed, such as gender, age, preoperative serum CEA level, tumor location and size, histological type, gross type, pathological grade, infiltration depth, lymph node metastases, pelvic or peritoneal metastases,Dukes staging and extrahepatic metastases. Results: Of 494 rectal cancer cases, there were 33 cases with hepatic metastases, accounting for 6.68%.Univariate analysis showed that tumor size, involving the perimeter, infiltration depth, pathological grade, pelvic or peritoneal, lymph node metastases and Dukes staging were associated with hepatic metastases.Multivariate Logistic regression analysis showed that lymph node metastases and Dukes staging were independent factors affecting hepatic metastases. Conclusion: Lymph node metastases and Dukes staging were independent factors affecting their hepatic metastases from rectal cancer.
