Angioscopy-Assisted In-Situ Saphenous Vein Bypass

In recent years considerable interest has been directed toward in-situ saphe nous vein arterial bypass as the best method of revascularization. But this pro cedure requires adequate exposure of the saphenous vein to allow valve incision and venous branch ligation.The authors used a LeMaitre valvulotome to find the superficial venous branches under direct angioscopy assistance. They found this procedure ideal for performing in-situ venous bypass surgery, for the following reasons: (1) It is a simple, time-saving technical procedure. (2) Small skin incisions have caused no problems with postoperative healing. (3) It is easy to locate competent or nondisrupted valves, perforators, and branches. (4) There is a decreased rate of arteriovenous fistulas after in-situ vein bypass graft.
