For newly detected round lesions of the lung a primary tumor or metastases of the lung have to be ruled out at first step. If primary malignancy is already evident, metastases is the most likely diagnosis. Histological evaluation and complete staging is of major significance for proper multimodal therapeutic concepts. Comprehensive therapy for pulmonary metastases depends on histology of the primary malignancy and its tumor spread. Time from initial treatment of primary tumor to detection of lung metastases is crucial. Their number and location are of great importance for the therapeutic considerations. For solitary pulmonary metastases of epithelial and non epithelial malignancies radical resection is treatment of choice.
