Abstract This case report describes a typical case of hemodynamically significant mid-cavitory obstruction and the well-recognized limitation of transthoracic echocardiography in diagnosing apical thrombus. It also highlights the importance of an imaging modality like CMR for unequivocal diagnosis and designing the management strategy.
