ABSTRACT Pigmented villonoduler synovitis is a proliferative disorder of the synovium and deposition of hemosiderin into the lining tissues of joints, tendons and bursae. PVNS is further classified into diffuse and localized forms, the most affected joints are the knees, hips and fingers. Shoulder involvement is extremely rare. In this paper, we present diffuse villonoduler synovitis of the right shoulder of a 45 year old male. The diagnosis is made by pathognomonic MRI findings and histopatho-logical analysis of the lesions. The joint involvement in this case is of monoarticular type.
