Effects of retroperitoneal laparoscopic ureterolithotomy and ureteroscopic lithotripsy on elderly patients with upper urinary calculi

Objective To compare the effects of retroperitoneal laparoscopic ureterolithotomy (RPLU) and ureteroscopic lithotripsy (URSL) on elderly patients with upper urinary calculi. Methods A total of 64 elderly patients with upper urinary calculi admitted to the First People’s Hospital of yangquan from June 2013 to April 2018 were selected as the study subjects. According to surgical methods, they were divided into RPLU group (n=32, treatmented by RPLU ) and URSL group (n=32, treated by URSL). Perioperative indicators, quality of life, inflammatory stress level and postoperative complications were compared between the two groups. Results The operative time in RPLU group was longer than that in URSL group, and the hospital stay in RPLU group was significantly lower than that in URSL group (P 0.05). After intervention, the scores of physiology, psychology, environment and social relationships of WHOQOL scale in the two groups were significantly increased (P<0.05); and the scores of four items in RPLU group were higher than those in URSL group (P<0.05). The inflammatory stress indexes of interleukin-6, endothelin-1 and prostaglandin E2 were significantly increased in the two groups 1 d after operation (P<0.05), but the above indexes in RPLU group were lower than those in URSL group (P<0.05). The overall incidence of complications in RPLU group (9.38%, 3/32) was significantly lower than that in URSL group (31.25%, 10/32), P<0.05. Conclusions RPLU for elderly patients with patients with upper urinary calculi has the advantages of high stone clearance rate and short hospital stay. And it has less impact on patients and few complications, which can effectively improve quality of life.
