7135 Patients outcome after injection hemostasis with epinephrin (ep) 1/10000 or combination ep + ethanolamine oleate 5 % in ugi hemorrhage from bleeding ulcers.

Aim: detection the hemostatic results of injection hemostasis in bleeding ulcers and comparison the efficacy of two agents for injection therapy. Patients: 259 (male 196,female 63),mean age 64.3 years with UGI bleeding from ulcers with major hemorrhagic stigmata. Method: Emergency UGI endoscopy, injection hemostasis in ulcers with major hemorrhagic stigmata with A: EP (in 167 pts) or B: EP+EO (in 92 pts) and follow up of patients outcome. EO injected direct into vessel 2-3 ml in bulbe ulcers and 2-5 ml in gastric. Results: Group A 167 / B 92 pts (active bleeding 85 / 38 [spurting 9 / 7],non bleeding visible vessel 82 / 54):Hemostasis initial 161 pts (96.4%) vs 90 (97.8%),permanent 148 (88.6%) vs 84 (91.3%),Blood units/pt 2.47 vs 2,08, (in visible vessel 2.6 / 1.8),= 4 units/pt 48 (28.7 %) vs 22 (23.9%),Operation 22 (13.2 %) vs 8 (8.7%),Deaths 12 (7.2%) vs 5 (5.4%). No serious complication (in operation had coverted perforation 3/2 pts). Conclusion: Endoscopic injection haemostasis with EP 1/10000 or Ep plus Ethanolamine oleate 5 % solutions is simple,safe and has great success (initial >96 %,permanent = 90 %). Injection of Ep + E.O. 5% had better results (no statistically significant) in pts with non-bleeding vessels and it is not so dangerous (as beleved) especially in gastric ulcers cases.
