PO-GG09 : Impact of age on long-term outcomes of midurethral sling surgery in female

Objective: Trans-obturator tape(TOT) and trans-vaginal tape(TVT) are a minimally invasive surgical management for stress urinary incontinence(SUI). The aim of this study was to investigate long-term outcome of TOT and TVT are in different age groups. Methods: Medical records of patients who underwent TOT at Obstetric gynecology department and TVT at Urology department from January 2010 to December 2015 were reviewed. Study population was stratified into three groups according to patients age. All patients were interviewed at baseline before surgery and after surgery within 4 weeks and further interviews were given if there were any symptoms. Primary outcome was recurrence of leakage symptom after surgery. Secondary outcomes were hospital stay, duration of operation, blood loss in surgery, and complication. Results: Total 571 patients diagnosed SUI and 262 patients who had not performed concurrent procedure were enrolled in this study. There were 81 patients in group 1 with patients under the age of 50 years, 97 patients in group 2(between 50 and 59 years), and 84 patients in group 3(60 years and older). Older age groups had higher prevalence of hypertension, DM and higher BMI. Group 3 MUCP values were lower than those other groups. After surgery, there were no differences between three groups in recurrence of urinary incontinence rate according to interview(3.7% vs 7.2% vs 2.4%, p=0.298). Also there were no difference between three groups in operation time(31±10 vs 31±13 vs 30±9min. p=0.638), blood loss(15.8±30.2 vs 17.02±5.0 vs 11.2±5.2cc, p=0.217), hospital stay(3.03±0.50 vs 3.09±0.85 vs 3.16±0.83 days, p=0.542) and complications rate (17.3% vs 19.6% vs 16.7%, p=0.498). When analyzing TOT group(217 patients) and TVT group(45 patients) separately, primary and secondary outcomes were did not differ significantly according to age groups. Conclusion: As surgical management for SUI, midurethral sling surgery, both TOT and TVT were found to be similarly safe and effective technique across different age groups.
