Prognostic value of histologic subtyping in completely resected stage I lung adenocarcinoma.

Objective:To investigate histologic subtyping in completely resected stage Ⅰ lung adenocarcinoma(AC) and determine whether AC subtypes have any prognostic significance.Methods:From 2001 to 2004,one hundred fifty-three patients with stage Ⅰ adenocarcinoma underwent completely resection and were subtyped according to 2004 World Health Organization Pathological Classification Standard.All patients were followed up after surgery.Data were analyzed using SPSS 11.0 software.Results:The mixed subtype of AC was most common out of the 153 specimens(n=82,53.59%).Sixty-one patients(39.87%) demonstrated a pure AC subtype.They included 42 cases(27.45%) of papillary AC,9 cases(5.88%) of acinar AC,6 cases(3.92%) of BAC,and 4 cases(2.61%) of SACM.Other 10 patients showed AC variants including 9 cases of muscinous AC(5.88%) and 1 case of fetal AC(0.65%).The 3-year survival rates postsurgery were highest in both pure BAC and fetal AC(100%) and lowest in SACM(45%);but the survival difference between them was not significant(P=0.330).There were no significant differences in the tumor size,pleural invasion,and TNM staging between the AC patients with(9 patients) or without(144 patients) SACM component.The 3-year survival rates were highly different between the two groups(54.55% vs 79.22%,P=0.048).Conclusion:Adenocarcinoma with an SACM component predicted poor prognosis after completely resection for stage Ⅰ AC patients.
