In summary low plasma sodium levels often observed in the elderly may be due to: (1) a certain degree of incompetence of the ascending limb of Henle's loop, (2) low aldosterone level and a decreased response of the renal tubule to this hormone, (3) abnormal intracellular shift of sodium.
