When controlling potentially confounding factors like maternal age, parity, gestational age at delivery, severity of hypertension, blood pressure levels and association with HELLP syndrome during pregnancy, the most important factors associated with postpartum maternal complications were diagnosis of HELLP syndrome before delivery (OR=13.5; 95%CI = 2.19–83.43) and cesarean section (OR=9.38; 95%CI = 3.02–29.11). Conclusions: Maternal morbidity is significantly higher among preeclamptic patients who had a cesarean section, independent of labor. Labor induction should be considered a reasonable option in management of these patients and the diagnosis of severe preeclampsia per se cannot be considered an indication for cesarean section.
