The analysis on the risk factors of recurrence of Cushing's disease after transsphenoidal surgery

Objective To investigate the risk factors affecting the recurrence of Cushing's disease after transsphenoidal surgery.Methods A total of 87 patients with Cushing's disease admitted in our unit from 1997 to 2003 were analyzed retrospectively.Predictive risk factors including sex,age,course,preoperative hormonal(level),preoperative MRI finding,tumor size,operative methods,histological finding and postoperative hormonal levels were investigated.The data were dealt with t-test,χ~(2)test,rank sum test,and multivariate Logistic regression analysis.Results Recurrence of Cushing's disease occurred in 21 of 87 patients with a mean time of 33.5 months(ranged from 12~60 months).The univariate analysis demonstrated that age,operation methods,histological finding and postoperative hormonal level were significantly related to recurrence.The multivariate(Logistic) regression analysis showed that operation methods(OR=3.993,P0.05),histological finding(OR=0.200,P0.05) and postoperative hormonal level(OR=5.280,P0.05) were the risk factors affecting the(recurrence) of Cushing' disease after transsphenoidal surgery.Conclusion Operation methods,histological finding and postoperative hormonal level are the significant independent predictors of recurrence for Cushing' disease after transsphenoidal surgery.
