What's your diagnosis? A 52-year-old woman with a subcutaneous, slightly movable and painless nodule in the left breast.

Human dirofilarial infestations have been increasingly reported from many parts of Mediterranean Basin in recent years. We present a case of subcutaneous breast nodule in a 52-year-old Croatian women, which is also a third reported case of human dirofilariasis in this country. It was clinically presented as a well circumscribed nodular lesion, which was hypoechogenic on ultrasonographic examination. The patient was treated by surgical excision and histopathologic analysis revealed transverse sections of female filarial nematode of the genus Dirofilaria, enclosed in inflammatory nodule. Because the vast majority of the cases are discovered in tissue sections, the dermathopathologists should be acquainted with the characteristics of the parasite to render accurate diagnosis.
