Clinical Characteristics of Coronavirus Disease 2019 Patients with Maintenance Hemodialysis/ 需维持性血液透析患者合并新型冠状病毒肺炎的临床表现

Objective To analyze the clinical characteristics of the coronavirus disease 2019 ( COVID-19) patients with maintenance hemodialysis ( MHD) . Methods A retrospective analysis was performed on 78 COVID-19 patients with MHD admitted to the Fifth Hospital of Wuhan in Wuhan,from February 13,2020 to March 3,2020. They were divided into critical group ( n = 12) and general group ( n = 66) according to the severity of the disease,and patients were followed up to the clinical endpoint. The observation indicators included total blood count,lymphocyte count,C - re- active protein ( CRP) ,procalcitonin ( PCT) ,B-type natriuretic peptide ( BNP) ,chest CT and nucleic acid detection. Results 82 1% MHD patients combined with COVID - 19 had atypical or no clinical symptoms. The positive rate ( 51 2% ) of nucleic acid detection was significantly lower compared with chest CT results ( 71 7% ) ( P < 0 05) . Com- pared with the general group,the CRP [( 33 53 ± 10 28) mg /L vs. ( 157 46 ± 45 57) mg /L],PCT [( 0 256 ± 0 035) μg /L vs. ( 1 362 ± 0 874) μg /L]and BNP [( 2 838 7 ± 359 6) ng /L vs. ( 25 007 6 ± 2 355 9) ng /L]were significantly higher in the critical group;and the lymphocyte count [( 0 95 ± 0 87) × 109·L -1 vs. ( 0 35 ± 0 08) × 109·L-1] was extremely lower in the critical group ( P < 0 01) . Conclusion MHD patients combined with COVID-19 have mild clinical symptoms,and are associated with a higher risk of mortality. Adequate hemodialysis can improves prognosis.
