Estrabismo y diplopía en paciente con alta anisometropía e intervenido de joven de desprendimiento de retina

espanolSe solicita la opinion de cinco expertos estrabologos sobre el caso clinico de un paciente de 51 anos de edad que acude por vez primera a consulta en diciembre de 2017 por presentar estrabismo ojo izquierdo y diplopia desde hacia 4-5 meses. Se les informa de los antecedentes sistemicos y oculares, de las pruebas que aporta y de aquellas que se le solicitan; asi como de la exploracion, cirugia que se le realiza para su estrabismo, resultado y situacion actual. Y se les pide su opinion acerca de las siguientes cuestiones: 1. Resumen breve del cuadro ante el que nos encontramos y causa de la recidiva. 2. ?Como explicas que ante una agudeza visual tan baja en ojo izquierdo, el paciente tenga diplopia? Y, ?ante unas condiciones sensoriales y motoras tan poco favorables siga con capacidad de unir en ocasiones las imagenes de ambos ojos? 3. ?A que crees que se debe el aumento progresivo, tras la cirugia de D.R., de la hendidura palpebral izquierda, especialmente a costa del parpado superior? 4. ?Efectuarias o solicitarias alguna otra prueba? 5. ?Realizarias de nuevo tratamiento quirurgico o esperarias? 6. En el momento de optar por la cirugia: ?Que tipo de anestesia emplearias? ?Cual seria tu planteamiento quirurgico? Aparte del TDP, ?tendrias en cuenta algun dato peri-operatorio para cambiar o decidir el protocolo quirurgico? ?Tenderias a hipercorregir para evitar de nuevo la recidiva? Se exponen los comentarios completos de todos los panelistas y se hace un resumen final de los mismos. EnglishThe opinion of six strabismus experts was requested for the clinical case of a 51-year-old patient who came to the clinic for the first time in December, 2017, complaining of strabismus of the left eye and diplopia presenting the previous 4-5 months. They are informed of the systemic and ocular past history, the test results the patient has provided and any other test requested, as well as the examination, the surgery performed for his strabismus, results and current situation. And they are asked to give their opinion on the following questions: 1. Brief summary of the condition we are facing and the cause of the recurrence. 2. How do you explain that, with such low visual acuity in the left eye, the patient complains of diplopia? And, given such an unfavorable sensory and motor situation, the fact that the patient is still capable of occasionally joining the images of both eyes? 3. What do you think is the cause of the progressive enlargement of the left palpebral fissure, after RD surgery, especially due to the upper eyelid? 4.Would you carry out or request any other tests? 5. Would you perform surgical treatment again or would you wait? 6. When opting for surgery: What type of anesthesia would you use? What would your surgical approach be? Besides the passive duction test, would you consider any perioperative data to change or decide the surgical protocol? Would you tend to over-correct to avoid recurrence again?
