Chest Pain Caused by Myocardial Ischemia or Not: Sometimes the Old Handgrip Test Can Solve the Dilemma

We describe the case of a patient with typical chest pain but negative maximal bicycle exercise-electrocardiogram test and for whom significant coronary artery disease was hypothesized by a positive handgrip exercise test and demonstrated by coronary angiography. Despite negative exercise stress test, handgrip as well as other provocative tests have to be considered when the pretest probability of coronary artery disease is intermediate and the symptoms are typical for angina. RESUME Nous decrivons le cas d’un patient chez qui une maladie coronarienne significative avait ete suspectee par un test positif au dynanometre manuel et confirmee par une angiographie coronarienne. Ce patient presentait une douleur thoracique typique mais un electrocardiogramme negatif lors de l’epreuve d’effort maximale sur bicyclette. En depit d’une epreuve d’effort negative, tant le test au dynamometre que les autres epreuves de provocation doivent etre consideres lorsque la probabilite pre-test de maladie coronarienne est de niveau intermediaire et que des symptomes typiques d’angine sont presents.
