Infantile status epilepticus as a complication of near-miss sudden infant death

SUMMARY Among 13 infants with apparently cryptogenic status epilepticus occurring between two and four months of age, two shared several features of ‘poliodystrophy’ encephalopathy. For the other 11, an anoxic-ischaemic event was suggested by the history, the seizure and EEG characteristics, and clinical and CT-scan evolution. The possible relationship with ‘near-miss’ sudden infant death is discussed. RESUME Etal de mal epileplique infantile el mon subile ‘vue de pres’ du nourrisson Parmi 13 nourrissons ayant presente un etat de mal sans cause apparente entre deux et quatre mois d'âge, deux d'entre eux presentaient plusieurs des caracteristiques d'une encephalopathie ‘poliodystrophique’. Pour les 11 autres, un accident anoxique ischemique etait suggere par l'historique, les caracteristiques cliniques et EEG de la crise, l'evolution clinique et au scanner. La relation possible avec une mort subite ‘vue de pres’ du nourrisson est discutee. ZUSAMMENFASSUNG Status epilepticus und abortiver plotzlicher Kindstod Von 13 Kindern mit vermutlich cryptogenem Status epilepticus, der im Alter zwischen zwei und vier Monaten auftrat, hatten zwei mehrere Merkmale der ‘Poliodystrophie’ Encephalopathie. Bei den anderen 11 Kindern wurde aufgrund der Anamnese, der Anfalls-und EEG-Charakteristika und des klinischen Verlaufs und der CT-Scan Entwicklung eine anoxisch-ischamische Ursache angenommen. Eine mogliche Relation zum abortiven plotzlichen Kindstod wird diskutiert. RESUMEN Estado epileptico infantily ‘near miss’ muerte subita infantil De entre 13 lactantes con estado epileptico aparentemente criptogetic entre los dos y cuatro meses de edad, dos presentaron caracteristicas de encefalopatia poliodistrofica. Para los otros 11 casos la historia sugirio un episodio anoxico-isquemico y establecio relaciones entre las caracteristicas entre la convulsion y el EEG y la evolucion de la clinica y la TAC. Se discute la posible relacion con la muerte subita infantil ‘near miss’.
