CONSERVATIVE TREATMENT OF AN EXTENSIVE MAXILLARY LESION IN A PREGNANT PATIENT

A 19-year-old pregnant female patient was referred for our service due to an extensive intraosseous lesion in the left maxilla. Extra-oral examination revealed swelling of the left middle third of the face with evolution of 8 months. Intra-oral examination showed swelling of the left hard palate and superior vestibule between the incisive and canine. On vitality test lateral incisive was nonvital. Radiographic and computed tomography exams revealed an extensive and well-defined radiolucent image on the left maxilla with extension for the maxillary sinus and part of the left nasal cavity. Based on physical and imaging examination the clinical diagnosis was a periapical cyst. Conservative treatment by decompression was first performed, and the histopathologic exam confirmed the diagnosis of periapical cyst. Ten months later the maxillary lesion was significantly reduced, and the enucleation followed by periapical surgery were performed after childbirth. The patient is being followed up at our oral medicine clinic.
