The ongoing process of atrophy and remodeling of alveolar bone, observed after the loss of dental elements, directly affects the bone availability, generating limits for correct placement of implants for anatomical interference neurovascular bundle. In some cases the inferior alveolar nerve replacement is essential to obtaining a good morphologic and functional rebalancing of the jaw. This study aims to report a case in which it was held alveolar nerve transposition technique lower than the implant installation. Patient 43 years old, female patient was toothless in posterior mandible with severe atresia rim. The treatment was performed under local anesthesia, through the transposition of the inferior alveolar nerve followed by implant installation. Despite the risks of neural dysfunction, the transposition of NAI in cases of severe atresia jaw, it is necessary to obtain a prosthetic rehabilitation with adequatemorpho-functional balance, which enables greater longevity of implant-prosthesis set as well as improves the patient's quality of life.
