Enfoque terapéutico con técnica de Monaldi en una bula con nivel hidroáereo

Introduction: The bullae is defined as the air space greater than 1 cm in diameter, without epithelial wall, which can be seen in the emphysematous lung parenchyma in 80% or in the parenchyma of healthy subjects in 20%. It is a form of localized disease and does not necessarily indicate the presence of generalized emphysema. Complications that can occur include infection, pneumothorax, and bleeding. In cases of infection, an image of a thin-walled cavity with air-fluid level appears on the chest radiograph. Treatment is antibiotic therapy for a period of 6 to 8 weeks. In case of torpid evolution or lack of therapeutic response, surgical intervention could be considered. Objective: To describe a case of an infected bulla with hidro aereal level, treated through Monaldi Technique therapeutic approach. Case report: We report the clinical case of a patient with an infected bulla that did not respond to antibiotic treatment, therefore Monaldi technique was performed. In the literature reviewed there is little evidence of the performance of this technique in infected bullae, and it is more frequently used in lung abscesses. Given the patient's situation, as well as his functional ventilatory compromise, the therapeutic option was chosen at Hermanos Ameijeiras Surgical Clinical Hospital and the patient had satisfactory evolution.
