Ensuring Quality Transitions of Care from the Hospital to the Home Setting by Utilizing Virtual Visits: A Novel Pilot Study

Background and Objectives: Ensuring quality transitions of care is challenging. Multiple post-discharge issues including missing appointments and prescriptions have been identified in large numbers of hospitalized (Rehm et al. J Hosp Med. 2018). Benefits of post-discharge telemedicine, or virtual visits (VV), on these issues have not been previously studied in a general pediatric population. In this multi-phase study, we created and assessed the impact of a VV pilot program on post-discharge care.
