Anatomic repair for the surgical treatment of simple transposition of the great arteries should preferably be conducted before one month of age , after this period a gradual decline in pulmonary vascular resistance which reconditions the role of the left ventricle ( LV) occurs a circuit of low pressure with decreased muscle mass , a situation that may cause failure in the immediate postoperative VI unable to take systemic circulation sharply . We present two infants , which could not be operated before the month of age, the two had not trained the left ventricle ( platanito ) and decreased ventricular mass, they are both place an occlusion of the pulmonary artery and a systemic fistula lung , and in a second stage anatomical correction . Both survived ventricular training and are able to perform anatomical correction . The option to "train" the left ventricle and its ability to withstand asidevolverle systemic output is puedelograr increasing afterload by an occlusion of the pulmonary artery and pulmonary systemic short to alleviate the causes cyanosis .
